  • Please enter the four digit reference number for the booking under which you are a guest.
  • We require the Booking Guest to complete this form. Thank you for your cooperation.
    Mill Meadow strongly recommends that suitable travel insurance is acquired to cover circumstances beyond the Guest's control such as, but not limited to, jury duty, incarceration, change in personal or work circumstances, military service, illness (including Covid-19 and shielding), family emergencies and travel delays. Covid-19 is a known risk and it is now possible to insure your holiday against it. Please check with your insurer on the extent of the cover provided to ensure that it suits your needs. 
    Mill Meadow offers the option for guests to purchase Booking Protect Refund Protection at the time of booking. Booking Protect is a third party provider and guests should read the terms and conditions and understand the cover provided before adding the Booking Protect Refund Protection option to a booking. Please read this statement from Booking Protect regarding Covid-19: https://bookingprotect.com/coronavirus-guidance/ Mill Meadow is unable to assist you with any information relating to Booking Protect Refund Protection.     Booking Guests are required to confirm that they and their party are familiar with and will abide by the HM Gov guidance relating to Covid-19 and will provide any verification information requested of them by Mill Meadow. When booking multiple households into a single property, please consider the risks around any possible HM Gov restrictions (such as the Rule of 6) and how they might affect your ability to use the accommodation as originally intended. Please keep in mind that Mill Meadow enters into a contract with the Booking Guest to provide them with accommodation. Mill Meadow will work with the Booking Guest to ensure that they and their party meet the current HM Gov guidance. At the time of the stay, HM Gov guidance may impact the Booking Guest's original plans and guest list. Please visit for more essential information related to Covid-19 Compliance.
  • Immigration (Hotel Records) Order 1972 We are required to register the full name and nationality of all guests who will be over the age of 16 at the time of their stay. Please enter this information in the text box above. For those who are not British, Irish or Commonwealth guests, we are also required to register a passport number and place of issue (or other document which shows their identity and nationality) and details of their next destination (including the address, if known) on or before departure. Diplomats and their family and staff do not have to register. We are required by law to retain this information for 12 months on our booking platform and in accordance with Data Protection legislation.

      For site security and Covid Compliance, Mill Meadow is required to verify the identity of the Booking Guest upon arrival. Please upload a copy of your Drivers License or Passport.
    • For security purposes, we require the registration numbers of all vehicles associated with the booking. Unauthorised vehicles will not be allowed access. Changes must be confirmed in writing before arrival. If you are do not have this information at the time of booking, please enter TBC and then contact Mill Meadow to provide details before arrival. 
      Please visit https://millmeadow.co.uk/covid-declaration-id/ and complete one Covid-19 Declaration Form for each guest over the age of 18. Day Visitors should complete https://millmeadow.co.uk/day-visitor-covid-declaration/. Guests and visitors under 18 years of age should be listed in the table provided on page 2 of the form along with a contact email or phone number. Please return the complete the forms at least 7 days before arrival. Guests will not be allowed on the Mill Meadow site without a completed form.
